The following information pertains to the HealthReturns program for members enrolled in the Inspire and Inspire Plus Terms and Conditions.
HealthReturns has been specifically created to encourage members to embrace healthier lifestyle choices. This program rewards members for every positive step they take toward enhancing their mental and physical health. All members of the Momentum Medical Scheme, with the exception of those enrolled in the Ingwe and Fusion Options, can access Multiply Inspire at no cost and earn HealthReturns. Additionally, members have the option to upgrade to Multiply Inspire Plus for a monthly fee starting at R195, which allows them to increase their potential rewards. The primary distinction between these two offerings lies in the amount of HealthReturns that can be earned and the variety of partner benefits available.
It is important to note that HealthReturns will only be disbursed if the member is an active beneficiary of the Momentum Medical Scheme during the month in which they qualify for the rewards. Members can begin earning HealthReturns after a waiting period of three months. However, it is crucial to understand that the costs associated with health assessments and any necessary additional tests will not be covered by the scheme.
Members can accumulate HealthReturns by engaging in a variety of activities, which include:
• Completing the Lifestyle Quiz, which assesses members' current lifestyle choices and health awareness;
• Undergoing a health assessment conducted by a health professional affiliated with Multiply or through the Multiply app, allowing for a convenient digital option;
• Participating in either an in-person fitness assessment or an online assessment with a qualified health professional. For members under the age of 40 who do not present any health risks, there is the option to complete both a digital health screening and a digital fitness screening via the app. The results of the fitness level assessment remain valid for three months, enabling members to instantly access the highest partner rewards based on their Healthy Heart Score and fitness level;
• Meeting weekly and monthly activity goals, as well as recharge goals, which promote consistent engagement in physical activity; and
• Engaging in leaderboard challenges that foster a competitive spirit among members.
HealthReturns earned will be credited to the member’s HealthSaver account. Furthermore, members can also receive partner cashbacks from selected partners, which will also be deposited into their HealthSaver account.
Members have the flexibility to utilize their HealthReturns at various healthcare providers, including general practitioners and dental professionals, as well as at select approved retailers such as Dis-Chem, Clicks, DNAlysis, and SportSA. They can also shop with Multiply's online partners, which include well-known brands like Fitbit, Garmin, Polar, Suunto, Chris Willemse Cycles, Cycle Lab, SweatGear, and The Golf & Racket (Wilson and Bridgestone) and The ProShop.
Step 1: Completing the Lifestyle Quiz
Upon completion of the Lifestyle Quiz, members will receive a score ranging from 1 to 5. This score directly influences the monthly HealthReturns awarded to them, as outlined below:

Step 2: Completing a health assessment to secure HealthReturns
After finishing a health assessment, members will receive a monthly reward based on their results. The first reward will be issued on the day of the assessment, followed by monthly rewards for the next eleven months.

Important conditions to keep in mind
• Members will only receive rewards for their activity once Multiply has obtained the complete set of results necessary to calculate their Healthy Heart Score. This includes cholesterol, blood sugar, blood pressure, height, weight, and waist circumference, along with their smoking status. For members who undergo assessments at the gym, it is essential to note that these assessments typically do not include cholesterol or blood sugar measurements and thus do not qualify as a full health assessment until all required test results have been submitted.
• It is the member's responsibility to ensure that Multiply receives the results of their health assessment. If a member has their health assessment performed at a Clicks, Dis-Chem, or MediRite pharmacy clinic, the results should be sent electronically to Multiply. However, if the assessment is conducted at a general practitioner’s office or another pharmacy, the results may not always be submitted automatically. Members can also choose to submit their results via email to healthreturns@momentum.co.za.
• The health assessment results remain valid for a duration of 12 months. After this period, members will need to undergo another assessment. The 12-month validity begins from the month in which the assessment was conducted. Members have the opportunity to update their individual results to enhance their Healthy Heart Score during this timeframe.
• The Health Platform benefit provided by the Momentum Medical Scheme covers one health assessment annually for each adult beneficiary.
• Depending on the results of a member's health assessment, additional tests may be necessary. Should a member require further tests, such as glucose or cholesterol tests, a notification letter will be sent to them. Members can also access this information by logging into the Multiply app or visiting multiply.co.za.
• For any further glucose and/or cholesterol tests, members must present the pathology form at their nearest pathology laboratory. This form will be emailed to them and can also be downloaded by logging into momentummedicalscheme.co.za. Momentum Medical Scheme will cover the costs of these tests, except when a member is in a general waiting period.
• Members can earn HealthReturns during the three-month waiting period; however, the costs associated with health assessments or additional pathology tests required during this time will not be covered by Momentum Medical Scheme.
Adhering to clinical protocols
• This applies solely to members who are registered on a chronic management program for a condition listed in the Chronic Disease List. Members must consent to be registered on this program when prompted by the disease management team. If a member chooses not to provide consent for registration, they will not be eligible to earn HealthReturns.
• A member will be deemed compliant if they take their chronic medication as prescribed.
• The compliance check involves reviewing whether any relevant claims related to chronic conditions for which a member is registered were submitted in the last three months. Therefore, for any given month, a member will be considered compliant with treatment protocols if a claim for chronic medication is received by the medical scheme within this month or the previous two months for all chronic conditions they are registered for. If members pay out of pocket and do not submit the claim, or if they claim for medication not specified for their condition, they will not be considered compliant and will not receive HealthReturns.
• Based on the results of a member's health assessment, additional tests such as cholesterol or glucose pathology tests may be recommended. If further testing is necessary, a benefit authorization will be automatically created. Once the results of these additional tests are received, a member will be classified as compliant, and their Healthy Heart Score will be recalculated based on the outcomes of the pathology tests.
Step 3: Completing a fitness assessment to earn additional HealthReturns
The results of the fitness assessment will determine the amount of HealthReturns a member can earn for the duration of its validity, as well as the level of partner rewards associated with their Healthy Heart Score. After completing a fitness assessment, members will receive a monthly reward (with the first reward issued on the day of the assessment and subsequent rewards on the first day of the following eleven months).
While a fitness assessment is not mandatory, members who complete it can significantly enhance their rewards. Members can have their fitness assessment conducted through Multiply’s network of health professionals. If they prefer, members can also complete the fitness assessment online from the comfort of their own home, which will be valid for six months. In contrast, an in-person fitness assessment will remain valid for twelve months. For members under 40 years of age without health risks, there is an option to complete both a digital health screening and digital fitness screening via the app. The results of the fitness level assessment will be valid for three months, allowing members to instantly access the highest partner rewards based on their Healthy Heart Score and fitness level.
The cost of an in-person fitness assessment is R480, while an online assessment costs R270. Members can pay for these assessments using their HealthSaver account. It is advisable for members to confirm the cost of the assessment with the health professional beforehand. To view a complete list of health professionals, members can log into the app.

Step 4: Engaging in physical activity and recovery (Active Dayz and Recharge Dayz)
Weekly Wins
Members can earn weekly rewards, termed Weekly Wins, by achieving their personal Active Dayz goals for that week, as detailed in the following table. Members who have completed a fitness assessment may have the potential to earn even more if they reach a fitness level of 4 or 5 as validated by the assessment. Active Dayz can be earned through daily steps and calories burned; however, gym visits, park runs, and participation in events do not contribute to Active Dayz. To track their activity, members must utilize a Multiply-approved fitness device or app.
An active day is defined by completing one of the following activities:
• Walking 10,000 steps in a single day;
• Burning 300 calories or more in one exercise session.
If multiple activities are performed on the same day, the first event that meets the criteria will be used for tracking.

Members who achieve Weekly Wins can further boost their earnings by completing Recharge Goals (through earning Recharge Dayz). To qualify for recharge rewards, members must first meet their weekly activity goals. For instance, if a member's weekly goals include achieving two Recharge Dayz and four Active Dayz, they will earn recharge rewards if they meet both sets of goals.
A member’s Recharge Score reflects their physical and emotional readiness to tackle daily challenges. This score takes into account sleep recovery and body stress by measuring heart rate variability (HRV), which indicates whether the body is under strain and requires additional mental recovery and rest. Members can determine their Recharge Score through a screening test available on the Multiply app. This can be done by scanning their finger with a phone’s camera before 8:00 AM each day. If multiple scans are performed before this time, only the result of the first successful scan will be considered for determining whether a member qualifies for a recharge day.
If a member's Recharge Score is high (indicating they are Super healthy or Healthy), they will receive a recharge day. Conversely, if their Recharge Score is low, Multiply provides resources such as the Digital Coach tool and partners like Calm to assist members in managing their mental and physical health.

Monthly Wins
Members who successfully achieve their personal activity goals for four consecutive weeks will be rewarded with a Monthly Win. Additionally, members who also meet their Recharge Goals over the same four-week period will receive a boosted monthly reward. Members who have completed a fitness assessment may earn more if they achieve a fitness level of 4 or 5 as validated by the assessment.

Step 5: Engaging in leaderboard challenges
The leaderboard feature allows members to compete directly in various health-related activities, including step challenges, calorie contests, and stress-free days. Members can earn rewards for participating in only one leaderboard challenge per month, and the reward is determined by their ranking within that challenge.

Multiply kids fitness assessment
The kids fitness assessment is available to members of the Momentum Medical Scheme who are enrolled in the Incentive, Extender, and Summit Options and are also members of Multiply Inspire Plus. This assessment provides insights into a child's developmental progress and highlights areas for improvement. Rewards are based on participation rather than outcomes, and parents receive recommendations on how to enhance or maintain their child's health.
The assessment is available for children aged 7 to 11 and 12 to 17 years. It is free if conducted during the same session as the parent's fitness assessment. If the child undergoes a separate assessment or a second assessment, the cost is R250. Children will receive a Nu Metro voucher or a similar incentive on the day of their assessment for participating.
HealthReturns of R50 will be credited monthly for a period of 12 months based on participation in this program.
Please note
Members will only earn HealthReturns if their benefits are deposited into their HealthSaver account. If members do not possess a HealthSaver account, Multiply will accumulate HealthReturns for a maximum of three months and will only disburse them once the HealthSaver account has been activated. If the HealthSaver account is not activated within this timeframe, all accumulated HealthReturns will be forfeited. Should the HealthSaver account be suspended, Multiply will continue to accumulate HealthReturns for three months to allow the member time to reinstate the account; after this period, any HealthReturns will be forfeited. All HealthReturns credited to a member’s HealthSaver will be stored in a HealthReturns Booster balance, which will be forfeited if the member terminates their membership in the Momentum Medical Scheme or their HealthSaver account.
All HealthReturns benefits will be credited to the member's HealthSaver account on the first day of each month following the month in which the activity was recorded. This is subject to system constraints and may be delayed. It is the member's responsibility to ensure that all activities are updated promptly, as failure to do so may result in not earning HealthReturns, and any back payments will be at Multiply's discretion.
Members will not earn interest on their HealthReturns. The maximum amount of HealthReturns that a member can earn monthly is limited to 40% of their medical scheme contribution for those on Multiply Inspire Plus and 10% for those on Multiply Inspire.
Fraudulent or inappropriate use of HealthReturns
If Multiply suspects that there is inappropriate or fraudulent use of HealthReturns, or if it believes that a member or their dependents are abusing the benefits or privileges of the program, or are misleading or defrauding the program, Multiply reserves the right to terminate any associated rights or benefits immediately and reclaim all funds disbursed to members since the beginning of their membership in the program.
Multiply retains the authority to cancel or modify the HealthReturns Program and its associated benefits at any time as deemed necessary. While every effort has been made to ensure that the information provided is accurate and up to date, this cannot be guaranteed. Any use of this information by third parties is entirely at their own discretion and is factual in nature. Multiply does not accept liability for any loss, damage, costs, or expenses that may arise directly or indirectly from any errors or omissions contained in this document.

It is important to note that Momentum is not a medical scheme and operates as a separate entity from the Momentum Medical Scheme. The complementary products are not classified as medical scheme benefits. Membership in the Momentum Medical Scheme is possible without enrolling in any of the complementary products.
Please be aware that the amendments to the benefits and contributions of the Momentum Medical Scheme for 2024, including the introduction of the new Fusion Option, have been submitted to the Council for Medical Schemes (CMS). The proposed changes are currently awaiting approval from the Registrar and are therefore subject to such approval. The Scheme is in discussions with CMS regarding the registration of the new Fusion Option and is awaiting a final decision from the Registrar.
